Prescription Request Form

Please note: If your pet has not had a veterinary check in the last 6 months or is unstable on its medication, we are unable to provide a repeat prescription until we have examined them. This is a legal requirement for all prescription medications.

If you require medication for multiple pets please submit a separate request for each one.

From 5th August 2024, we require 3 working days’ notice for all repeat prescriptions placed before 1pm. If you require items to be posted to you, postage costs of £6.04 will apply.

Please note that prescription requests are monitored and actioned Monday to Friday.
